Southlake Health Navigator PRIVACY POLICY

Protection of Privacy

At Southlake, we strive to serve with purpose, and we recognize that your information deserves to be treated with respect and sensitivity. The Southlake Privacy Office takes great care in upholding the compliance requirements of Ontario’s privacy and health-privacy legislation; the Freedom of Information and Protection of Privacy Act (FIPPA), and the Personal Health Information Protection Act (PHIPA). The Southlake Health Navigator (also "the App", the "Navigator") respects the privacy of its users.

You should not use the App if you do not agree with this Privacy Policy. If you any concerns related to privacy and the use of this App, we encourage you to reach out to the Southlake Privacy Office at 905-895-4521 ext. 2395 or via email at privacy@southlakeregional.org. All incidents will be handled in a confidential manner. If you are not satisfied with the response of the Southlake Privacy Office, you may wish to escalate your incident to the Information and Privacy Commissioner of Ontario.

User Behaviour

Southlake reserves the right to perform statistical analysis on user behaviour and characteristics associated with App. This aggregated information will be used for quality improvement (e.g., to measure interest in and use of the various tiles of the App to improve design and navigation) as needed. Only aggregated data from these analyses, and not personally identifiable data, will be used for this purpose.

Cookies

Some features of the Navigator use cookies that are stored with your device’s browser storage. A cookie is a small text file sent from a Web server and placed on your computer’s hard drive. A cookie is generally used to enhance your browsing experience. You can choose to set your browser to detect and reject cookies, to accept cookies from all sites, or to prompt you whenever a site wants to send you a cookie. If you choose, they may be used to 'remember' your password and make it easier and faster to log-in to certain sites. Check your browser’s "Help" files to learn how to do this. If you choose to refuse cookies, you may not be able to access some of the interactive features of the Navigator or on our websites.

Cookies used by the Navigator, do not give Southlake access to anything on your hard drive and cannot affect your device(s). Southlake applications use two types of cookies: session cookies (temporary) or persistent cookies (longer-term continuing use).
